What Does the Exercise Referral Scheme Involve?
Our Exercise Referral instructors will find out what is important to your patient and help them find activities they will enjoy. A 6- and 12-week review enables the instructor to support the individual through their first 12 weeks towards a healthier lifestyle. Support and reviews continue to be offered for up to 12 months following their referral to encourage long-term behaviour change.

Activities vary at each centre but may include:

Walking sports or led walks

Group exercise classes (such as chair-based exercise)

Casual swimming or water-based activities (such as Aqua)

Gym-based exercise following a programme set by our coordinators

Social groups, including coffee mornings for increased peer support

Condition-specific activities, such as Good Boost technology for MSK conditions

 

Scheme Exclusion Criteria
Our team at Everyone Active Danes Camp, Lings Forum, and Mounts Baths are Level 3 Exercise Referral trained, meaning they are qualified to support low- and medium-risk clients to start their fitness journey.

Please note: An individual may not be re-referred onto the scheme within 12 months of previously participating.
If an individual presents with any of the following contraindications to physical activity, they will not be accepted onto the Exercise Referral Scheme, and we will contact the referring practitioner directly:

Unstable angina or ongoing ischemic chest pain at rest

Decompensated / acute heart failure

New or uncontrolled or symptomatic arterial or ventricular arrhythmia

Severe symptomatic aortic stenosis

Acute myocarditis, pericarditis, or endocarditis

Acute systemic infection with fever or significant systemic illness (temperature >380)

Uncontrolled hypertension: typically resting SBP ≥180mmHg and/or DBP ≥100mmHg

Significant orthostatic hypotension with symptoms (e.g. SBP drop ≥20 mmHg with symptoms)

Acute pulmonary embolism / pulmonary infarction / acute DVT

Acute aortic dissection or large/symptomatic aortic aneurysm (e.g. AAA >5.5 cm or symptomatic)

Severe hyperglycaemia (blood glucose ≥19mmol/L) or recurrent severe hypoglycaemia (blood glucose ≤4mmol/L), thyrotoxicosis/myxoedema not controlled

Unstable sternum or recent major surgery not cleared for exercise

Severe rejection in cardiac transplant recipients

Physical disability or condition that precludes safe exercise testing/participation

Active psychiatric instability that prevents safe participation (in some specs)

ICD patients with recent shocks or un-interrogated device activation until reviewed

Patients with significant unexplained BP drop during exercise testing

Those who fail exercise tolerance testing with marked ischaemia, complex arrhythmias, or severe symptoms
