To register: SIMPLY FILL IN OUR FORMS AT RECEPTION.
You will be asked for the name of your last doctor, so we can request your medical hstory.
As soon as your forms are completed you will be asked to make an appointment with our HCA to complete your registration. Only after this appointment will your registration be complete. You will then be able to book an appointment to see a Doctor or nurse.
We can help you fill in the forms.
Medical notes can take a long time to be forwarded from your previous doctor, it is therefore very important that you attend this appointment as it gives the doctor some baseline medical information.

Please note that submitting a registration form does not mean you are registered with the practice as an appointment will need to be made with our HCA.
Alternatively print off a registration form, fill it out and bring it with you on your first visit to the practice.

If you are ill while away from home
you can receive emergency treatment from the local GP practice for 14 days.
After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months.
This will allow you to be on the local practice and still remain a patient of your permanent GP.
After three months you will have to re-register as a temporary patient or permanently register with that practice.
